Fields
Certificate number:

Enter the reference number from the employee's medical certificate.

Certificate Date:

Enter the date that the employee's medical certificate was issued.

Degree of incapacity:

Select the degree to which the employee's injury affects their capacity to work.

  • (Blank)
  • P-Partial
  • T-Total.
Incapacity status:

Select the extent of incapacity caused by the employee's injury.

  • (Blank)
  • P-Permanent
  • T-Temporary.
Incapacity From:

Enter the date from which the employee was unable to complete any work duties. This date includes work time taken by the employee to obtain their medical certificate.

Partial Return Date:

Enter the date that the employee is expected to return to partial work duties.

Full Return Date:

Enter the date that the employee is expected to return to full work duties. The employee's actual return date is recorded on the Return to Work tab.

Note: PayGlobal calculates the Details(ii) tab | Weeks Absent value from the Incapacity From and Full Return Date values.

Certified By:

Select the health professional who issued the employee's medical certificate.

Certifiers Comments:

Enter the health professional’s comments from the medical certificate.
