First aid provided:

Yes/No field that indicates whether first aid was given to the injured person.

First aid provided by:

Enabled when First aid provided = Yes.

Type the name of the company and/or person who gave the injured person first aid.

Medical attention:

Yes/No field that indicates whether medical attention was given to the injured person.

Medical attention provided by:

Enabled when Medical attention = Yes.

Type the name of the company and/or person who gave the injured person medical attention.

Date:

Date that the injured person received First Aid or Medical attention.
