Summary

The Accidents Monthly Report contains the following information:

  • Injured person’s name
  • Sub division
  • Supervisor
  • Date of incident
  • Location
  • Category
  • Injury type
  • Body part
  • Treatment
  • Lost time
  • Hours lost
  • Significant hazard.

Report Settings

Use the Period Start Date and Period End Date to set the reporting period.

Use the Query tab options to modify your report by:

  • Body part
  • Date of incident
  • Incident category code
  • Treatment type code.
